Care Coordination Assistant/ Part Time/ Evening Shift - CORPORATE

Facilitates and coordinates patient
throughput based on information from Social Work and Case Management
Departments

1. Performs coordinative duties for healthcare
   team for discharge planning and facilitation.

• As
  directed by healthcare team, coordinates discharge and post discharge
  services (community services, medications, prescriptions, transportation,
  etc.).
• Utilizes
  clinical discharge plan to help identify appropriate resources based on
  patient needs, level of care, coverage and availability.
• Assists
  in referral process by transmitting required patient documentation using
  web-based software system.
• Communicates
  with insurance companies to facilitate care and secure authorization, as
  required.

1. Performs diversified administrative duties supporting the
   operational needs of Case Management and Social Work Departments

• Assists
  healthcare team members in using and updating department computer system
  to keep track of patient activity.
• Compiles and
  analyzes information summarizing departmental activities and prepares
  statistical data for reporting purposes, as requested by management.

1. Performs
   related duties as required.

• High School Diploma or equivalent, required. 
• Minimum of two (2) years progressive responsibility in  discharge planning, insurance verification,
  financial counseling, and/or medical billing, required.  
• Knowledge of medical terminology, required.
• Working knowledge of computer applications, required.
• Knowledge of third party insurance authorization
  requirements, preferred.
• Experience using case management and/or third party
  billing applications, preferred.
• Ability to communicate effectively, required.
