We currently have a full-time Care Coordinator position available in our Comprehensive Community Services Program (CCS).

Position Summary: This position is responsible for helping our individuals understand, navigate, and grow through the CCS program. This Care Coordinator meets and works with the individuals, service providers, and other team supports that assist in the individuals progression through the CCS program. The Care Coordinator receives consultation from mental health and substance abuse professionals while working on the individuals individualized recovery plan.
Essential Functions:
- Establish individuals eligibility by using a Mental Health/AODA Functional Screen.
- Assist the individual in identifying services from the CCS service array appropriate to address recovery needs.
- Broker services as needed with other providers in the CCS network.
- Work with the individual to identify and build a recovery support team of professionals as well as natural supports.
- Work with the individual to make an individualized recovery treatment plan.
- Facilitate recovery meetings with the individual and the team.
- Work with the individual to prioritize and obtain the goals set forth in the individual recovery plan.
- Follow-up with service providers to ensure quality care and goal progression.
- Appropriately document all contact with individuals and team members.
Other Duties and Responsibilities:
- Ensure individuals can get transportation to all appointments that are CCS focused.
- Other job-related duties as may be necessary to carry out the responsibilities of the position.
Job Qualifications:
Knowledge, Skills and Abilities: An understanding of mental health, substance use, and co-occurring needs and the ability to clearly communicate these to others; ability to support with emergency situations; the ability to maintain and promote positive working relationships within WCS and work openly and effectively with ones direct supervisor and colleagues; excellent assessment and interviewing skills; strong understanding of mental health and substance use needs; ability to work effectively as a member of a team; effective use of time and resources to help maintain quality CCS services. Experience working with Milwaukee County Behavioral Health Services preferred.
Minimal Qualifications:
- Bachelors Degree in Social Work or a related field required. Masters Degree preferred.
- Two (2) years of experience working in the Mental Health field required.
- Experience working with people with substance use needs preferred.
- Meet all WCS requirements for criminal and caregiver background checks and drivers record.
- Valid drivers license, automobile, and sufficient insurance to meet agency requirements are required.
Other Job Information (if applicable):
Work Relationship and Scope: Reports directly to the CCS Supervisor (MHP); works with the Clinic and Unlimited Potential psychiatrist, nursing staff, the CCS treatment team as well as the benefits advocates, acts as a liaison with landlords, employers, community medical staff and family members as needed.

Working Conditions: Work is performed in the office, the community and individuals homes; hours average 40 hours per week. Drivers license and vehicle required.
Physical Demands: Position is mobile with some time spent in the community, including occasional transporting clients, home visits and attending meetings; driving throughout Milwaukee County; must be able to go up and down stairs.
