CARE COORDINATOR POSITION SUMMARY:

The Care Coordinator works with public agencies, healthcare teams, and human service providers. Local Behavioral Health Authority and other community partners to successfully connect patients/clients to community-based resources/services across Maryland.

EDUCATION AND EXPERIENCE:

• Bachelor’s degree in or a related field and five (3-5) years of related experience.
• Work experience may substitute for education requirements, and years of experience in social work, public health, healthcare, or human services.
• Experience providing social work services to developmentally delayed, psychiatric, and/or substance use patients.
• Experience with Maryland’s behavioral health system.

POSITION RESPONSIBILITIES INCLUDE:

• Provide full coverage of the 211 Maryland Hospital Care Coordination referral line Monday – Sunday, 8 am – 8 pm
• Receive and respond to incoming phone and electronic behavioral health patient referrals from Maryland hospitals for service placement at community-based organizations, within 30 minutes.
• Utilize the 211 Resource Database to immediately identify available services, gather key information to make connections with service providers, and provide warm handoffs to service providers on behalf of patients.
• Provide follow-up services within 48 hours of patient placement to ensure patient connection to services and monitor for additional risks.
• Regularly communicate the progress of patient resource placements with referring hospitals.
• Thoroughly document care coordination activities including patient data, resource placements, and follow-up activities in Maryland Information Network’s dedicated technology tools.
• Securely maintain confidential patient records, assessments, and related documents.
• Attend program-related training and meetings with 211 Maryland.
• Escalate cases to the state and communicate and update patient activities in the MIN patient portal.
• Work with hospitals on the 211-boarder list chart, develop reports on the data, and communicate with the supervisor.
• Provide weekly or monthly reports.
• Perform other duties as assigned by the supervisor.

Software: Google Suite (required), Microsoft Office Suite (required), iCarol CRM System

ADDITIONAL INFORMATION:

Work is currently conducted remotely. There may be times for in-person meetings, community functions, or other events that attendance will be required. Occasional walking or standing and interaction with personnel located in other parts of the building are also necessary when in the office.
