• Hourly Rate: $29.65 per hour

Duties:

Provide care coordination services for the Minnesota Army National Guard in support of surge and contingency requirements as well as its steady-state mission. Contract personnel shall not perform inherently governmental functions (i.e., representation of government, decision authority, etc.) Responsible for tracking medical status of ALL assigned cases, or Soldiers as required.

• Complete the work of receiving, communicating and processing the
  medical and personnel records that are submitted in all the various e-systems.
• Scan administrative and medical documentation into HRR and ensure it is titled appropriately no more than one week after receipt.
• Support Case Managers with the below duties
  • Prepare and ensure Reserve Component Managed Care (RCMC/ARCP) cases are administratively prepared and submitted for Soldiers with low risk, low acuity continuing medical conditions resulting from an approved LOD illness, injury or disease.
  • Monitor and update trackers (LOD tracker, eCase, MED-DET Non-Duty, PEB tracker, Temporary Profile tracker, INCAP tracker or MED tracker) with the medical and dental status of Soldiers and ensure any medical or dental related issues identified, are followed, documented, paid for if applicable, bills paid and case managed.
  • Complete medical charting, case processing, medical documentation, and
    profile concerns for MNARNG Soldiers in all required systems of record utilized by the MED-DET which are MEDCHART, MEDPROs, Medical Evaluation Board (MEB) Trackers, ND-PEB Trackers, Temporary Profile tracker, eCase and HRR. Update current status of case, contact information, suspense dates and case development will all be updated. Forward relevant medical documents, case processing and profile concerns to military or civilian medical providers. Maintain medical/dental office or hospital contacts for case updates and medical records requests
  • Track Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready to deploy or is administratively separated from the military
  • Maintain accuracy of all managed cases in applicable administrative trackers and ensure all administrative charting in eCase to include any contact with SM (via email, voice call, or personal interview) is completed within 24 hrs of the action occurring.
  • Request profile exams/evaluations and audiology exams at physical exam station.
  • Coordinate Soldier treatment plans with appropriate medical facilities. Submit case processing, medical documentation, and profile concerns to military/contacted provider and track medical appointments by communication with unit, and/or Solider to ensure Soldier attendance and submission of follow up documentation.
  • Maintain automated referrals indicated in MEDCHART.
  • Submit case processing, medical documentation, and profile concerns to military/contacted provider.
  • Attend PHA events as determined by the needs of the state and the individual units. Ensure those Soldiers referred by a provider are followed by the case management team. Manage Soldier’s case from the identification of the condition to when the Soldier is either:
    - Considered medically ready.
    - Administratively separated from the military.
  • Ensure all medical documentation received for any Soldier and/or their medical team is received and placed in appropriate electronic record, to include all lab work, and referrals.
  • Maintain soldier medical records in accordance with Army Regulation (AR) 40- 66. Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines. Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40-501/502 and other applicable guidance. 100% of the time.
  • Obtain release of information from Soldier.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) certification, standards and guidelines.
  • Review, upload and track health assessment (PHA, SRP, Specialty Physicals), lab results, and contact SM and educate on lifestyle modifications.
  • Upload medical documents of case managed Soldiers into Health Readiness Record (HRR). Stamp all medical documents for verification after uploading. Index uploaded medical documents.
  • MODS entry of immunizations, designation of pregnancy, eye exams, medical warning tags.
  • Provide all medical documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the contractor to the government. Copy of each supporting transfer document (DA form 200) shall be provided to the COR at the end of each month no later than 7 calendar days at the end of the month.
  • Contractor will ensure all administrative charting in eCase is completed.
  • Track medical appointments by communication with unit, and/or Solider to ensure Soldier attendance and submission of follow up documentation.
  • Responsible for medical charting in eCase, to include any contact with SM (via email, voice call, or personal interview), coordination with Soldier for treatment plans with appropriate medical facilities. Contractor will ensure all administrative charting in eCase is completed within 24 hrs of contact.
  • Communicate with delegated government personnel to track compliance versus noncompliance.
    Ensure Soldiers with profiles are case managed in the eCase module.
  • Attain and maintain Integrated Disability Evaluation System (IDES) MEB enrollment percentage in accordance with National Guard Bureau standard
  • Manage all IDES cases for SMs from assigned units no less than weekly. IDES cases will be reviewed either retuned for corrections or submitted to MEBTO within five business days from receipt. Contractor prepares a monthly report containing number of packets
  • Manage all LOD cases for SMs in assigned units no less than weekly. LODIs will be reviewed and either returned for correction or submitted to State Approval within three business from receipt. Contractor prepares a monthly report containing number of on time and overdue LODs completed
  • Manage all NDR cases of SMs from assigned units no less than weekly. NDR-PEB cases will be reviewed and either returned for corrections or submitted in MEB Prep within five business days from receipt. Contractor prepares a monthly report containing number of NDR cases validated.
  • Attend deployment SRP and FRU events to review temporary and permanent profiles, identify potential non-deployable Soldiers, review medical cases, have Soldiers sign a ROI if necessary, inform them about the boarding process, identify any medical billing issues and counsel them on the process to get their bills paid. Update medical status and waivers in MOB tracker with
    current and new cases and collect necessary paperwork to update or start profiles that would impact deployability. Provide findings to the Minnesota Medical Detachment and unit attending SRP.
  • Manage all required waivers for assigned deploying units on no less than a weekly basis.
  • Manage Hearing cases for all assigned units.

Qualifications:

No educational requirements. Preferred experience in the Army or National Guard. Must be proficient in MS office suite and general administrative duties.
