About the Job
Join the Gastroenterology team at 87th Ave as a full time Care Coordinator ! As a Care Coordinator you will provide complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients on the daily schedule. Working from a team approach, and in continuous partnership with patients and providers to promote timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being. Pay range starts at $24.00 and goes up based on evaluation of experience. Schedule is Monday through Thursday, 8:00a-5:30p and Friday, 8:00a-12:00p. Possibility of working remote after successful completion of full time, on-site training and meeting the remote worker* requirements.
In this role you will:
- Serve as an effective communication link between patient and clinic staff by gathering information from patients.
- Independently and proactively identify patients in the practice by running panel reports for specific diagnosis and preventive care needs.
- Evaluate and analyze patient records for accurate pre-visit data. Communicates with patient directly to discuss preventative care needs and refers patients with medical concerns to appropriate clinical staff.
- Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
- Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients.
Requirements:
- WA State Medical Assistant-Certified or Registered
- Required are completion of one of the following: active Medical Assistant-Certified credential from Washington State Department of Health or combination of experience and training resulting in a nationally recognized certification (AAMA, NCCT, AMT, NHA) or completion of a formal MA program or formal medical services training program of the United States Armed Forces or EMT training and certification. Must obtain Washington State Medical Assistant credential within required timeframe and maintain in active status throughout employment.
- Current Healthcare provider CPR certification, American Heart Association BLS.
- At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
* Remote worker must live in local Vancouver, WA or Portland, OR area and have secure home network with download speed, min 25 Mbps and upload speed, min 5 Mbps. Remote work is not guaranteed.
Pay Range:
$23.51 - $32.91
