Care Coordinator- Hospice and Home-Based Palliative Care - Nationwide Children's Hospital

The Care Coordinator assists patients and families with complex medical, palliative, or hospice needs through transition to home and hospital settings. The Care Coordinator will support in-home nurses by handling day-to-day coordination of needs from the office setting. The coordinator is key in helping the experience be as seamless as possible for patients and families.

0.5 FTE, 20 hours/week

• Communicates with the patient’s physician team, orders medical equipment and supplies, medication refills, and collaborates with discharge planners as adjustments as complex transitions are made from hospital to hospice or in home palliative care.
• Develops self-management skills through education and resource provision. Build relationships with hospice and palliative care patients and families through time on the program.
• Supports families with all circumstances, including use of interpreters. Adjusts level of support needed based on assessment of family’s abilities or complexity of the child’s medical needs.
• Visits patient homes when needed to enhance the coordination experience for the family.

Education: BSW, RN, or related field, required.

Licensure:
• Must maintain a current Ohio license.
• Valid Ohio driver’s license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.

Skills:
• Working knowledge of Medicaid and other regulatory agency standards, preferred.
• Strong organizational and attention to detail skills, required.
• Robust written and verbal communication skills, required.

Experience: Experience working in a patient facing role in healthcare, preferred.
