Position Information:
The Care Coordinator RN is a Registered Nurse who is assigned to the Financial Clearance Department to facilitate the physician care plan for a patient's hospital encounter.  Using a collaborative interdisciplinary approach the Care Coordinator will confirm the plan of care and focus on anticipation integration and coordination of clinical care necessary to meet the needs of patients in order to optimize clinical and resource outcomes.  The Care Coordinator collaborates with Physician offices Physician Advisors Point of Entry Case Managers Unit-Based Case Managers and Case Management leadership in assessing patient needs and developing the plan of care.   The Care Coordinator will also act as an advocate for the patient physician and hospital relative to compliance with federal state and third party utilization management regulatory requirements

Experience Requirements:
3 Years Nursing - Related Area Required - In Clinical - Case Management Or Clinical - Utilization Management Preferred

Education Required:
Graduate Nursing Required - And Bachelor's Degree  Required - In  Nursing Preferred

Licensure/Certification Requirements:
Registered Nurse Required - And Certified Case Manager Or Commission for Case Management Certification Preferred

Hours of Work:
9:00am - 6:00pm (4 days/week)

Weekend Requirements:
No Weekends
