Care Coordinator at West Side Health Care District

 This position would be support for:

Health Plan driven quality programs, HEDIS measurements, other internal and external quality programs, patient education and data management.

The Care Coordinator will work with the Care Coordinator Supervisor to implement patient care plans and assist with provisions of all necessary services for the patient’s medical, behavior and social needs.

Required: ONE of the following must apply

1. Bachelor’s Degree from and accredited university in a field of study such as Public Health, Public Administration, Business Administration, Computer Science or other applicable field with at least (2) years of experience working in health care or related field.
2. Possess an active license as a Medical Assistant and worked for a least (2) years in a health care setting.
3. Worked as a Case Manager in a health care setting for at least (2) years.

Other Requirements and Preferences:

• BLS/CPR Certification (required)
• Bilingual Spanish/ English – Healthcare Versed (preferred)
• Case Management (preferred)
• Active Driver’s License and Proof of state required Auto Liability Insurance (Required)
