Join our client as a Care Delivery Supervisor in New York, NY, overseeing clinical services at participants' homes in either Manhattan or the Bronx, ensuring care plan compliance. Compensation: $110,000 - $120,000/year
Location: New York, NY (Fieldwork in either Manhattan or the Bronx)
Schedule: Monday to Friday, 8:30 AM to 5:30 PM
Responsibilities as the Care Delivery Supervisor:
Join our client as a Care Delivery Supervisor in New York, NY, overseeing clinical services at participants' homes in either Manhattan or the Bronx, ensuring care plan compliance. Responsibilities as the Care Delivery Supervisor:
- Supervision: Guide and oversee Community Health Nurses (CHNs) and Home Health Aides (HHAs) to ensure quality home care.
- Assessments: Evaluate participant needs and develop comprehensive care plans in collaboration with interdisciplinary teams.
- Field Visits: Conduct joint visits with CHNs for supervision and competency evaluation.
- Documentation: Complete and submit timely and accurate documentation in compliance with federal and state regulations.
- Quality Assurance: Perform chart audits, participate in in-service education, and report on clinical changes or participant needs.
- Education: Bachelor of Science in Nursing (BSN) required.
- Experience: 1+ year in home care or community health with frail or elderly populations required (PACE or MLTC experience preferred).
- Licensure: Active and unrestricted RN license in New York State required.
- Skills: Strong organizational, presentation, and computer skills.
- Knowledge: Familiarity with NYS Department of Health regulations.
