Care Management Social Worker - Adult Inpatient - Maine Health

~ 40 Hours/week, Full-time, Day shift ~

Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is required

Position Summary

Maine Medical Center has an immediate opening for a Care Management Social Worker (Licensed Clinical Social Worker or Licensed Masters Social Worker - Clinical Conditional) to join our care team. This role will work with our adult inpatients. The schedule is typically Monday – Friday 7:30 AM – 7 PM and will include every 5thweekend and rotating holiday coverage (2-3 holidays/year).Case load is typically 1:40 with social work interventions not necessarily indicated on all 40. We can offer clinical supervision to new hires.

The Care Management Social Worker role is responsible for the ongoing assessment of the biopsychosocial-spiritual status of the patient and family/supports. The social worker will work collaboratively with the interdisciplinary team members, both internal and external to the organization to facilitate discussions regarding discharge options as they relate to complex situations, facility placement, or community resources in the context of the patient’s health insurance, available support, location and other eligibility factors.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Master of Social Work Degree from an accredited school required.
2. License/Certifications: Licensed by the State of Maine as LMSW or LMSW-CC required.
3. Experience: Two years of experience in a hospital setting preferred.
