Care Manager – Emergency Department –The Doorway at Concord
Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a Care Manager to join The Doorway at Concord in working with clients with a Substance Use Disorder.
The Care Manager is a person who is grounded in peer-assisted recovery. They will be a member of a multidisciplinary team, spending half of their time working in the Concord Hospital Emergency Department. Because of their life experience with substance use and/or other addictions, the care manager provides expertise that clinical training cannot replicate. They provide psychoeducation and recovery coaching to clients. Care managers also provide essential expertise and consultation to the entire team to promote a culture in which clients’ point of view and preferences are recognized, understood, respected, and integrated into support, treatment, rehabilitation, and community-based recovery orientated systems of care.
Responsibilities:
- Provide support and engagement strategies to individuals who have presented to Concord Hospital Emergency Department.
- Support disposition and discharge planning of cases presenting with substance misuse at Concord Hospital Emergency Department.
- Provide early intervention to clients who have relapsed or whose recovery is at risk.
- Provide follow-up calls to clients who have presented to the Concord Hospital Emergency Department.
- Participate in client data gathering as required by the state of NH which includes the GPRA interviews.
- Maintain contact with clients until they are successfully engaged with external providers.
- Provide outreach services to clients when they appear to have not engaged following their call or have disengaged from Doorway contact.
- Provide care management services to clients of the Doorway and participate as an integral part of the Doorway team.
- Support the distribution and training for Naloxone to individuals and organizations in Merrimack County and surrounding towns.
- Maintain healthy boundaries to ensure a high quality and safe therapeutic relationship with clients.
Requirements:
- High School diploma or equivalent is required
- Lived experience with and knowledge of substance use treatment, recovery coaching, and other related human services is required
- Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative is required.
- Knowledge of the system of care for substance use and mental health treatment, as well as available resources in Merrimack County and surrounding areas preferred.
- Ability to successfully complete the credentialing process with Concord Hospital granting hospital privileges is required.
- Certification as a CRSW is preferred, willingness to work to obtain the certification is required.
- Excellent communication, problem-solving, time management, and organizational skills are required.
