As the Social Work Care Manager for Hoag Memorial Hospital, Newport Beach, you’ll be responsible for collaborating with Hoag medical staff and the community they serve to ensure an environment of extraordinary patient care. Being a Social Work Care Manager means coordinating appropriate post-acute services for the patient based on the clinical assessment by physicians, nurses, and ancillary staff. As the Social Work Care Manager, you will not only consider patients’ needs through the continuum of care, but will also offer psychosocial support to patients and families for adjustment to illness, grief counseling, and psychiatric stabilization.
Inside the Care Management Department
Our Care Management team provides stream-lined coordination of patient care services to Hoag patients and families. From admission to post-discharge, our Care Management team is there to assure care is appropriate, timely, cost-effective, and of high quality. We look to our Care Managers to fully support and provide the Hoag Experience for our patients. Other duties include:
- Eliciting clinical assessments on patient discharge needs from the interdisciplinary team which includes the physicians, bedside nurses, physical therapist, speech therapist, occupation therapist, pharmacist, wound care RN and others involved in the care of the patient.
- Developing a comprehensive discharge plan based on the clinician’s assessments and recommendations for ongoing care, and the patient’s health plan guidelines.
- Interviewing the patient to review financial, family and psychosocial support in order to identify any barriers to the discharge plan.
- Providing psychodynamic intervention, crisis counseling, and end-of-life/bereavement support to patients and families.
- Adhering to HIPPA guidelines upon communication and coordination of resources in the community to meet the discharge and psychosocial needs of the patient.
- Documenting pertinent patient issues, discharge plan, and contact information for the family and providers of post-acute care in the medical record.
- Educating patients and families on their healthcare options and connecting them with resources to meet their needs.
- Assisting patient to advocate for self to receive appropriate services.
- Appropriately utilizing resources during and post hospitalization.
- Participating in achieving metrics established by the hospital to meet CMS quality standards, HCAPS, Magnet, and other hospital initiatives.
- Knowledge of payer requirements for discharge.
- Understanding the responsibility associated with being a mandated reporter for elder, child, and spousal abuse.
- Completing mandatory stroke specific competencies as applicable.

- Master’s Degree in Social Work (MSW)
Preferred
- Minimum 1 year experience as a Social Worker in an acute healthcare setting
