Job Summary/Objective:
- The Care Manager plays a pivotal role in providing targeted case management and supports coordination for individuals seeking assistance in accessing various health and social services. The primary objective is to help clients design and implement goal-oriented and individualized strategies to obtain necessary support. The Care Manager assesses, plans, links, advocates, coordinates, and monitors services, ensuring they are delivered in a responsive, coordinated, effective, and efficient manner.
Essential Duties and Functions:
- Conduct comprehensive assessments to identify clients' needs and develop individualized plans of service through person-centered planning.
- Provide linkage and advocacy to help clients access health and dental services, financial assistance, housing, employment, education, social services, and other community-based supports.
- Offer ongoing coordination and monitoring of services to ensure they align with clients' goals and objectives.
- Maintain accurate and compliant records in both electronic and hard copy formats, adhering to agency requirements and standards.
- Participate in competency improvement activities, including credentialing, continuing education, implementation of evidence-based practices, peer review attendance, and other professional development practices.
- Complete required training in the core elements of case management and supports coordination.
- Work independently and resourcefully, applying self-directed initiatives to meet client needs effectively.
- Participate in corporate quality improvement and problem-solving activities.
- Ensure confidentiality and privacy within MDHHS, state, and federal guidelines.
- Utilize knowledge of CARF, CMS, MDHHS, and Managed Care standards.
- Undergo annual TB testing as required.
- Perform other duties as required.
Qualifications:
Educational
- Bachelor's or Master’s degree in a human services-related field, required.
- One year specialized training or experience in treating or working with a person who lives with a Mental Health Diagnosis (QMHP), required for those applicants who have a bachelor’s degree in a human services related field.
- Licensure in the State of Michigan, preferred, LLBSW/LBSW. Other behavioral health licenses are also accepted (LLPC/LPC, TLLP/LLP, LLMSW/LMSW).
Basic Qualifications
- One year of computer experience with proficiency in Word, Excel, and other electronic medical record software, preferred.
- Strong organizational, verbal/written communication, computer, analytical, and problem-solving skills.
Trauma Informed System of Care:
A trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers. The impact of trauma is broad and touches many life domains. Participation is mandatory for activities required for Summit Pointe to become a “Trauma Informed System of Care.”
Physical Requirements:
- Professional office environment that generates noise from computer equipment/peripherals.
- Possible exposure to health risks (violence and infectious diseases).
- Sometimes lifts, carries, or moves objects up to 25 lbs.
- May be required to work variable hours/shifts.
