Position Overview:
We are seeking a passionate and experienced Pediatric Care Manager (RN) to provide case management for our pediatric members (ages 0-21). This role involves telephonic outreach, home visits, and collaboration with families and healthcare teams to ensure optimal care for our members.

Key Responsibilities:

• Conduct case management for Tier 3 and 4 pediatric members as a Pediatric Care Manager (RN).
• Perform monthly or quarterly telephonic outreach.
• Complete home visits upon member enrollment and quarterly for Tier 3 members.
• Attend and participate in regional meetings and team activities.
• Document all activities in TruCare.
• Collaborate with team members via Teams and Zoom.

Performance Expectations:

• Manage a caseload of approximately 200 members (Tier 3 and 4 combined) as a Pediatric Care Manager (RN).

First Day Experience:

• IT training.
• Orientation with a preceptor to learn day-to-day operations, policies, and procedures as a Pediatric Care Manager (RN).

Qualifications:

Required Skills/Experience:

• Pediatric case management or pediatric nursing experience.
• Strong communication and teamwork skills.

Preferred Skills/Experience:

• Experience with TruCare.
• Knowledge of Community Care and Medicaid (AHCA).

Education Requirements:

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• 2-4 years of related experience.

Licensure:

• RN - Registered Nurse - State Licensure (Compact State Licensure accepted).

Preferred Counties:
Candidates residing in Washington, Holmes, or Jackson Counties are preferred.
