About the Job
The Care Management Assistant will provide support to the Vandalia Health Network Population Health team. This position will collaborate through direct patient outreach, communication with network organizations, and post-acute providers to coordinate care. The Care Management Assistant will document all care management activities in the approved care management system.
Minimum Qualifications:
High school diploma or GED. Post high school certificate as a Medical Assistant, LPN, or Nursing Assistant. One year of experience in a patient care setting as a Medical Assistant, LPN, or Nursing Assistant required.
