Caregiver/Med Tech
Caregiver (HCA/CNA):
Essential Functions and Responsibilities:
Assisting residents with ADL tasks as indicated in their service plans. This includes but not limited to: assisting with bathing, grooming, toileting, dressing, oral care, ambulation, transfers, serving and assisting with meals, feeding, housekeeping, bed making, and laundry
Caregiver must always observe and uphold resident rights
Positively supports residents with emotional and behavioral issues
Caregiver must assist other staff as needed on the floors
Assist residents to and from meals
Caregiver must support resident activity participation by assisting residents to/from activities, encouraging participation, and leading activities when asked.
Caregiver must be willing to work with staff from all departments to ensure quality of care for residents
Keep environment safe and clean for residents; performing housekeeping duties as indicate don cleaning schedule or assignment sheets.
Observe residents for safety and report any and all changes in resident condition effectively, accurately, and in a timely manner to LN on duty
Assist in development and revision of Service Plans by informing supervisor of current resident needs and preferences as well as successful service approaches.
Answers all resident pendant/pull cords immediately.
Carry and possess a working knowledge of community call/communication system
All other duties as assigned.

Must be neat in appearance, pleasant personality, be congenial to fellow team members, residents, families, and the public in general. Must be capable of using own judgement and initiative in daily duties.
Always demonstrates safe working practices.
Maintains a professional image to residents and fellow co-workers in words, actions and in appearance
Always maintains confidentiality and privacy of our residents and staff.
Relate to residents in a courteous, empathetic, and cooperative manner.
Must be a mature individual. Must possess the ability to multi-task in a fast-paced environment and perform a variety of tasks, often changing assignments on short notice.
Caregiver must have strong organizational skills, as well as the ability to prioritize and meet deadlines.
Must be detail and team-work oriented.
Must attend required in-services/workshops to further education of skills and community requirements.
Qualifications:
- Current CNA or HCA License.
Must be able to pass Criminal Background Check, Health Screening including TB test.
Must be eighteen (18) years old.
Maintains current CPR and first aid certification.
Ability to read, analyze, and interpret care plans.
Assisted Living and Memory Care regulations knowledge.
Ability to prioritize and organize assignments.
The employee shall be responsible for maintaining the integrity of Fields Senior Living
Physical Demands and Requirements:
Maintain ability to lift 50 pounds; to stand or walk throughout the vast majority of the work day; and to frequently push, pull, bend, twist and reach.
Must be willing to be active and on feet during entire work shift.
Involves light to heavy physical activity performing non-strenuous and strenuous activities.
Must have sufficient manual dexterity to work with the fingers and to reach/hand items.
Med Tech:
Essential Functions and Responsibilities:
- Provide residents with medication administration and treatment assistance and services according to the health care provider’s orders
- Med Tech must supervise residents who self-administer medication while following company and regulatory guidelines
- Medical Technician must maintain resident records regarding medication distribution and incidents; Observe residents for safety and reports any and all changes in resident condition effectively, accurately, and in a timely manner to LN on duty
- Assisting residents with ADL tasks including but not limited to: assisting with bathing, grooming, toileting, dressing, oral care, ambulation, transfers, serving and assisting with meals, feeding, housekeeping, bed making, and laundry
- Med Tech must always observe and uphold resident rights; assist residents to and from meals; Relate to residents in a courteous, empathetic, and cooperative manner
- Must be willing to work with and provide assistance to other staff from all departments to ensure quality of care for residents
- Documents pertinent information in residents charts and in other specified locations (e.g.; alert charting and incident reports)
- Medical Technician must communicate with physicians, via fax and telephone
- Keep environment safe and clean for residents
- Perform other related duties as assigned by supervisor
- Carry and posses a working knowledge of community communication system and EHR.
- Observes for and reports resident problems and reports such problems to supervisor

Qualifications:
- Must be able to pass Criminal Background Check, Health Screening including TB test.
- Med Tech must be eighteen (18) years old.
- Maintains current CPR and first aid certification
- Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations.
- Assisted Living and Memory Care regulations knowledge
- Ability to prioritize, organize and delegate assignments.
- Medical Technician must attend required in-services / workshops to further education of skills and community requirements.
- Successful completion of state certified medication aide/tech course.
- Ability to communicate clearly and effectively, verbally and in writing in standard English.
