We partner with PACE (Program of All-Inclusive Care for the Elderly) to help these programs care for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.
CareLinx by Sharecare helps PACE programs with healthcare professional staffing and in-home care services including transportation to medical appointments, recreational activities, and other senior care services.
Job Summary:
CareLinx by Sharecare is looking for Per Diem Caregivers to join our team.
The Caregiver provides services to PACE participants in adult day programming and in the home. As a caregiver, you will be responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants in a home care setting. The Caregiver will provide supervision and support with clients as directed and report on any new conditions or needs that may arise. You will be making a difference in people’s lives and creating meaningful relationships with clients.

Qualifications:
• Current CPR certification
• Must possess, as a minimum, a GED or high school diploma
• Experience in a long-term care facility or with a community-based geriatric program preferred
• Minimum of six (6) months of documented experience working with frail elderly population highly preferred
• Knowledge of the principles and techniques involved in relating to frail elderly persons
• Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants
• Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and responsiveness to participant requests
• Ability to follow directions, verbal and written instructions, and adapt work schedule under supervision
• Ability to utilize appropriate safety measures when providing care
• Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner
• Well organized, dependable, flexible, and resourceful
• Requires the use of a personal vehicle
