Position Summary
Determine client needs and eligibility for assistance, arrange for services. Work under the guidance of the supervisor, or director
Essential Duties and Responsibilities
- Excellent writing and verbal skills and ability to establish rapport.
- Ability to work with culturally diverse communities and to be culturally sensitive and appropriate.
- Provide calls to “no answers” as reported by the Home Delivered Meals Provider.
- Conduct Two Month Monitoring Calls.
- Provide accurate and appropriate information on services, benefits and entitlements to clients.
- Advocate and intervene with other agencies on behalf of client.
- Maintain complete, detailed and accurate records.
- Attend In-service/DFTA required trainings.
- Other duties as assigned by Supervisor

Minimum Qualifications
- High School Diploma with experience in community work or in the human services field preferred. Knowledge of benefits and entitlements for seniors.
- Computer Skills Required. Working Knowledge of Microsoft Word essential.
- Bilingual English/Spanish speaking strongly preferred.
