Non-Exempt position
Hours: 15 hours per week - Thursday and Friday 8am-4pm with ½ hour unpaid lunch
Pay Rate: $16.78 per hour.
GENERAL PURPOSE OF JOB
Ensure that all medically necessary appointments (as determined by physicians and the Registered Nurse) occur in a timely and effective manner with the associated record keeping done in an accurate manner. Perform in a manner that promotes cooperation with others, e.g. consistently demonstrate a positive attitude, remain open and adaptable to change, do what you say you will do and focus on productivity not activity.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Coordinate scheduling of all Residential Services medical appointments as required by individual resident's physicians and determined by the Registered Nurse based on a resident's need. Timing of regularly scheduled appointments must be within the parameters of the resident's insurance coverage allowances.
- Coordinate transportation to all medical appointments for residents with respect to above determined needs in conjunction with individual resident's day programming or supported employment and Residential staffing capabilities.
- During medical appointments present appropriate information to physicians and advocate on behalf of residents.
- Ensure that all information (written or verbal) from each appointment is accurately distributed in writing to the Registered Nurse, Residential sites and the resident's day program Registered Nurse in a punctual manner.
- Log, track and retrieve all medical appointments via computer. Properly file all medically related information in individual resident binders.
- Advocate on behalf of residents with all providers of services to ensure quality care.
- File accurate and timely program notes conforming to the policy and regulations.
- Coordinate scheduled maintenance for department vehicle, reporting any related issues and follow up regarding repairs per Coarc policy.
- Participate in department meetings as requested.

Requirements of the position:
- High School Diploma or GED (now called TASC) preferred
- Exceptional written, communication, and computer skills
- A valid driver's license.
- Ability to be certified in all required trainings
- Occasionally required to lift up to 50 pounds
- Able to work independently
- Be very organized and motivated
