About the Job
The Office of Multi-Cultural Services of the Hennepin County Human Services and Public Health department has an opening for a Case Management Assistant (CMA). This position will seek out, interview, and assist people in need of agency services to facilitate transactions in the community and county settings.

Location and hours:
This position is hybrid and will be performed both on-site at the South Hennepin County Human Service Center at 2215 East Lake Street, Minneapolis, MN 55407 and in community locations within Hennepin County. Work hours are Monday through Friday, between 8 a.m. and 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
- Offer clients assistance in completing the Broader Needs Assessment:
- Explain purpose of Broader Needs Assessment to clients.
- Refer individuals to specific programs and/or agencies for service.
- Solicit feedback from clients regarding resources.
- Contact resources to provide client feedback and learn about what they have to offer to clients.
- Visit individuals and families in their homes when necessary to explain and facilitate supportive services and resources available.
- Interact with schools, medical personnel and other agencies to obtain information. Share information with clients and assist them in making appointments.
- Present information on programs, benefits, and resources to neighborhood groups, schools, healthcare providers, and non-profit organizations.
- Support employees communicate Hennepin County services, client responsibilities, and cultural assimilation techniques.
- Assist in completing forms for program eligibility related issues, gathering verifications to assist in program eligibility determinations, and connecting individuals to health and social service resources.
- Provide education on healthcare systems and the use of public and privately funded healthcare options.
- Maintain accurate records of client interactions, profiles, etc.; prepare reports as needed.
Need to have:
- One of the following:
- Two years or more of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two years or more of college coursework in the behavioral sciences.
- A valid driver's license, access to reliable transportation, and the ability to obtain a Hennepin County driver’s permit for travel to community service providers and home visits throughout Hennepin County.
- Ability to communicate orally and in writing in Pashto/Dari and/or Amharic/Tigrinya.
- Experience working with:
- Public assistance programs.
- Residents enrolled in financial assistance and other assistance programs.
- Microsoft Office Suite (Excel, Outlook, Teams & Word).
- Client interviewing techniques.
- Knowledge of community resources and social services available to individuals.
- Effective interpersonal and writing skills.
- Ability to:
- Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
- Establish and maintain professional working relationships with individuals, staff, and the public.
- Work both independently and collaboratively as an effective team member.
