About the Job
Position Overview:
Under supervision of the Director of Organizational Effectiveness , the Rehab Units’ Case Management Assistant functions as the person responsible for supporting the Rehab Units’ Case Managers and the Rehab Units’ Discharge Planners in administrative functions, in accordance with applicable federal, state and local guidelines and regulations, and with the Jewish Home of San Francisco’s policies and procedures.
Essential Job Functions:

Under the guidance of the Case Manager, fax nursing and rehabilitation’s weekly progress notes to managed care companies contracted with the Jewish Home.

Complete Home Health referrals based on Physician’s orders by obtaining orders and calling Home Health agencies.

Fax medical records to Quality Improvement Organization (QIO) when patient appeals decision to end Medicare or Managed Care coverage.

Complete Durable Medical Equipment (DME) referrals by assembling packet of information, faxing documents and calling DME companies.

Maintain confidentiality of necessary information.

Other duties as assigned by Director of Organizational Effectiveness.

Assist in completion of referral packets including Assisted Living Facilities (ALF), by assembling packet of information and faxing documents.

Assist in completion of community referral applications e.g., In-Home Supportive Services (IHSS) and Paratransit as needed.

Other duties as assigned.
Qualifications, Skills, Abilities:

High school diploma or equivalent.

Exemplary interpersonal skills and ability to work cooperatively as a member of a team.

Ability to communicate effectively with patients and their families, and at all levels of the organization.

Excellent time management skills to organize and prioritize workload.

Cultural sensitivity of Jewish traditions

Demonstrated strong computer skills including MS Office, Excel, Word, and electronic health records.

Physical Demands:

Ability to lift, push, pull a minimum of twenty-five (25) pounds.

Pay Range $22.00 - $23.50
