- Prepares or assists with the preparation of a variety of regular and special reports to include the Department of Health (DOH) monthly report, grants reports, etc.
- Completes weekly documentation review. Assists with program evaluation activities monthly.
- Creates, tables, charts, and graphs, to display patient data and grant deliverables
- Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
- Develops templates for the Maternal and Child Health team in the EHR or other databases when needed.
- Assists with data management to ensure compliance with grant deliverables.
- Partners with staff from Community of Hope’s prenatal programs and other community partners, assesses prenatal women using the Parent Survey and assesses other tools for appropriateness for Healthy Start postpartum or home visiting services.
- Administers the Parent Survey and submits findings within 48 hours of completion to determine eligibility for program services and/or referrals in clients’ home or clinic environment.
- Compiles required data in the EMR and Healthy Start database (salesforce) to document assessment, eligibility and appropriateness for programs per policy and procedure. Ensures that prenatal patients are screened for Healthy Start prenatally or within three months of delivery.
- Communicates with patients in a respectful and culturally appropriate manner. Follows Language Access policies at all times.
- Makes referrals to case management, housing, behavioral health and other services as needed.
- Bachelor’s degree in Social Work (BSW) or related field or equivalent experience required.
- If applicant has a BSW/MSW degree, LSWA/LGSW or appropriate social work license required.
- Familiarity and experience with quantitative and qualitative data collection and analysis methodologies required.
- Demonstrated cultural competence in communicating with underserved, diverse populations. Experience working with families preferred.
- Strong interpersonal, customer service, communication and organizational skills.
- Demonstrated capacity to input data accurately required. Experience working with EHRs or database system required.
- Strong organizational skills required.
- Ability to work flexible hours, including weekends and evenings.
- Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.

About Community of HOPE:
Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For nearly 40 years, we have provided healthcare, housing, and supportive services for low-income, underserved, and homeless people in Washington DC. As a Federally Qualified Health Center, we provide medical, dental, behavioral health, and care coordination services for the whole family at three locations in DC. Community of Hope also has a strong emphasis on maternal and child health, with a midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope is also providing community walk-in COVID testing and COVID vaccines. Community of Hope is also one of the largest providers in DC of housing and support services for families experiencing homelessness.
