Case Management Nurse - Case Manager - Travel - (CM RN) - Onward

: RN case manager to do both utilization review as well as discharge planning for pediatric inpatient (acute care) at a Tertiary care facility that provides services for a 7 state region. Responsible for a set group of inpatients, evaluating patient status, utilization review, securing accounts, care coordination with the inpatient teams, and discharge planning.The Case Manager develops, implements and evaluates individualized patient care plans. The Case Manager collaborates with the patient, family, other healthcare team members, community providers, and community resources and payers, resulting in an appropriate level of care, placement, length of stay, follow up care and reimbursement.
