- Job Responsibilities
- Responsible for the coordination and facilitation of the plan of care.
- Assessment of complete chart and knowledge of patient's medical condition, financial status, emotional needsand support systems.
- Collaborates with the Medical Director, Director, Manager, and Supervisor of Case Management on patientswho are not meeting the regulatory requirements for hospitalization.
- Provides initial and concurrent reviews for approval of hospital stay for all patients. Notifies patients whenstay has been denied.
- Collaborates with the interdisciplanary team to ensure patient quality of care and safety of discharge.
- Provides education to physicians, staff and patients/families.
- Education
- Associate's degree in Nursing or Diploma in Nursing required. Bachelor's degree in Nursing preferred.
- Licensure/Certification/Registration
- Licensed as a Registered Nurse (RN) required. Eligible to practice nursing in the State of Kentucky required.Certified as a Basic Life Support (BLS) Healthcare Provider preferred.
- Work Experience
- 2 to 8 years progressive experience in a related field required. Experience in Case Management preferred.
- Skills and Attributes
- Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to workin a stressful environment and take appropriate action.
