What You'll Do:
As a Case Manager, you will collaborate closely with our clinical staff to assess client needs, develop individualized care plans, and ensure a positive treatment experience. Your primary responsibilities will include:
As a Case Manager, you will collaborate closely with our clinical staff to assess client needs, develop individualized care plans, and ensure a positive treatment experience. Your primary responsibilities will include:
- Developing and implementing care plans in collaboration with clients and treatment teams.
- Facilitating communication among clients, psychiatrists, and other key stakeholders.
- Assisting clients with discharge planning and coordinating aftercare services.
- Maintaining accurate case notes and documentation in EMR systems.
- Conducting weekly case management sessions and participating in treatment team meetings.
Your Qualifications:
- Experience: 3+ years in a mental health treatment center, social work setting, or related field, with a focus on discharge planning and EMR documentation.
- Education: High school diploma required; Bachelor’s degree preferred.
- Skills: Strong interpersonal, analytical, and problem-solving skills, along with a high level of emotional intelligence.
- Knowledge: Understanding of mental health disorders, treatment options, and community resources.
- Other: Ability to drive locally (up to 30%) and basic computer skills (word processing, spreadsheets, etc.).
