Definition
Under the supervision of the Program Director, the primary responsibility of this position is to coordinate and provide services to persons affected by brain injury. These services are delivered using a model of person centered community-based case management.
Distinguishing Characteristics
This position requires practicing person-centered, community-based case management principles within the context of the policies and procedures governing Brain Injury Connections of the Shenandoah Valley. The individual will:
- Identify resources in the community for persons affected by brain injury
- Develop referral sources
- Engage in outreach and networking to build referral sources, which initially constitutes a large component of the position
Responsibilities
- Case Managers maintain a caseload of at least 25 active clients and 5 follow-along clients based on a 40-hour work week. The caseload is prorated accordingly if an employee's full-time work hours fall between 32-39 hours per week.
- Guided by the mission and values of the organization, a full-time Case Manager shall have the following duties as prescribed by our person-centered case management focus. Other duties may be assigned by the Program Director or Executive Director.
Client Evaluation and Intake:
- Perform a comprehensive intake evaluation, including the completion of all required documentation.
- Obtain necessary records from outside sources to ensure a thorough evaluation.
- Establish a collaborative partnership with clients, their families, and caregivers.
- Clearly articulate clients’ and/or families’ needs and appropriateness for services, as needed.
- Complete a thorough client assessment, which includes evaluating the client’s strengths and weaknesses, family and community support systems, and a needs assessment covering all areas described in our policy and procedure manuals.
- Assess existing resources currently utilized by the client/family.
- Assess assistive technology needs and seek appropriate resources to meet these needs.
Case Management:
- Develop an individual service plan annually for each client, that is reviewed and updated every 90 days.
- Support clients in developing their own goals and plans using a person-centered approach based on the needs assessment.
- Identify, coordinate, and monitor goal-directed community services to meet clients' needs and wants.
- Visit each active client at least once every 30 days and each follow-along client at least once every 90 days.
- Maintain regular contact with all clients (minimum 30-day intervals), offering support and guidance, and assist them in maximizing their participation in the goal attainment process.
- Obtain information and make referrals for appropriate services.
- Obtain appropriate releases, understand confidentiality laws, and uphold these laws.
- Communicate effectively with clients, their families, other support systems, and professionals.
- Create and facilitate support teams, including referrals, scheduling meetings, and delegating duties.
- Advocate on behalf of clients and their caregivers.
- Negotiate services for clients in their communities professionally.
- Travel and provide services in clients' homes and/or a community setting of their choice.
- Expend resources within the client's funding budget and seek approval of expenditures from the Program Director or Executive Director.
- Maintain and document client information in a web-based database.
Outreach and Referrals:
- Participate in public awareness and community outreach activities
- Develop referral sources
- Attend health and resource fairs
- Provide in-service trainings
- Attend fundraising events
- Build coalitions
- Develop and/or facilitate support groups
- Provide Crisis Intervention Training for first responders
Professional Development and Continuing Education:
- Obtain Qualified Brain Injury Specialist certification within 6 months of hire.
- Obtain Certified Brain Injury Specialist certification within 2 years of hire.
- Participate in professional development activities and stay abreast of current/new information in the field.
Knowledge Base:
- Understanding of brain injury and its effects on children, adolescents, and adults, as well as their families and caregivers
- Knowledge of case management practices
- Familiarity with supportive counseling techniques
- Proficiency in medical terminology
- Awareness of community agencies and resources
Job Requirements:
- Basic computer skills
- Minimum of a Bachelor’s Degree required, including but not limited to a Human Services field. A Master's Degree is a plus.
- One year of experience in case management and/or professional licensure preferred
- Ability to work with minimal supervision.
- Valid driver’s license and access to a personal vehicle
- Physical mobility
- Ability to engage in complex problem-solving and reasoning
- Proficient in reading, writing, and proofreading at a professional level
- Certification as a brain injury specialist or a 24-month action plan to acquire this certification required
- Travel is required, including visits to client homes. Due to the community-based nature of case management, there may be minimal exposure to adverse environmental conditions, such as smoke and the presence of pets.
