Position Title: Case Manager (CW) ($20.40hr - $22.80hr)

Essential Functions of the Case Manager:
- Case Manager's time will be spent providing direct, in-person services according to established program time frames and protocols.
- To provide individual, parent, and family group counseling/case management services to clients in client homes, schools, offices, and other settings as appropriate within the context of outreach services and according to treatment plans.
- To complete and submit to the case file all required clinical documentation in a professional manner within established program expectations and time frames
- To complete child/adolescent and Home Based Safety Assessment and/or parent intake interviews and their subsequent dictation according to established program time frames and protocol.
- To achieve stated treatment goals with assigned youth or families as identified by program expectations.
Minimal Qualifications for the Case Manager:
- Bachelor's Degree in a human service discipline
- Mature person with sound judgment and represents an appropriate role model to youth and families.
- Possess knowledge of adolescent development and behavior.
- Display organizational skills and highly adept at managing multiple tasks and assignments.
- Successful team members are creative thinkers, are flexible in their approaches to people and projects, and are capable of working with diverse groups of the population.
- Must have and maintain a valid driver's license including vehicle insurance and inspection on any vehicle used for daily work and transportation of clients.
- Must receive a passing score on annual motor vehicle clearance to be eligible to operate agency vehicles.
- Proficient in computer knowledge & skills (i.e. Microsoft Windows software); familiarity with the operations of external databases and Google Suite is desired.
- Never have been convicted of any prohibited criminal or child abuse offense.
Preferred Qualifications:
- A year of CASSP experience
Physical Requirements for the Case Manager:
Sedentary work that primarily involves sitting/standing. X Occasionally
- Light work that includes moving objects up to 20 pounds. X Occasionally
- Navigating stairs and tight spaces (accessing older buildings possibly through fire escapes, narrow staircases, and tight walkways). X Occasionally
- Driving to include long distance (2-6 hours in one day). X Occasionally
