Pay Rate: $24/Hour
The Case Manager - Intensive manages a case load of adults with severe and persistent mental illness through the activities of planning, linking, monitoring, advocacy, and assessment.
MINIMUM EDUCATION & EXPERIENCE
BA/BS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university
1 year of experience working in mental health or related field
Valid FL driver's license
PREFERRED EDUCATION & EXPERIENCE
MA/MS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university
2 years of case management experience
Case Manager Certification (CBHCM) - Florida
KNOWLEDGE & SKILLS
Familiarity with 65E regulations, Baker Act, and Marchman Act
Ability to interpret and abide by confidentiality laws, abuse reporting guidelines, and client rights requirements
Proven track record of abiding by a code of ethical and practice standards
Basic counseling skills
Case management skills
Advanced communication skills, both written and verbal
Excellent organizational and time management skills
General typing and computer literacy skills
Proficiency in educating adults on illness, medication compliance, community resources, money management, and basic life skills
Experience with assessment and treatment planning
Capability to handle verbally or physically aggressive clients
Knowledge of psychiatric medication and community resources
RESPONSIBILITIES
CLIENT CARE
Conduct Assessment Interviews with clients
Accompany clients to and link them with community resources, as needed
Counsel clients and family members, where appropriate, on relevant clinical issues
Perform weekly home visits to all clients on case load
CLIENT ADVOCACY
Advocates for client, such as assisting with completing applications for services and discussing clients needs with providers
Coordinates care with psychiatrists, therapists, medication clinics as well as with external services such as hospitals, social service agencies, and housing agencies
Monitors treatment process to ensure needed services are provided on a timely basis
RECORD KEEPING & OTHER ADMINISTRATION
Attend staff meetings
Documentation, including progress notes, progress reviews, and other reports
Develop comprehensive service plans
Compose and type letters, correspondence, etc.
Complete staff activity reports, PB2 and functional assessment rating scales
Accomplishes locus assessment at admission and every six months, as needed to determine level of care
Achieve daily entries into credible (EMR) schedule
Performs all other duties as assigned
