SCOPE OF ROLE
Reporting to the Supervisor, Behavioral Health Services, the QMHS, Forensic will coordinate with the designated court/jail personnel, as well as the client’s clinical care team, regarding client care. They will support clients by helping to manage their behavioral health symptoms, set up appointments, educate on treatment plans, and linking incarcerated clients directly to benefits, services, housing, and resources when released. The QMHS will additionally maintain routine contact with probation and the court/jail personnel, as requested by the client. The QMHS, Forensic will act as an integral member of a client centered treatment team, ensuring effective and comprehensive service delivery. Their work location will be both at the Cuyahoga Jail and in the community, assisting clients post-release.
HOW YOU’LL SUCCEED:
- Diligently develop, prepare and present treatment program work material to clients and assist them in planning for re-entry into the community, and maintaining recovery post-release.
- Transport or arrange for transportation of clients upon discharge, if determined to be safe and necessary.
- Maintains clear communication and professional boundaries with all inmates.
- Collaborates effectively with client’s treatment team and any court/jail personnel involved with their care.
- Encourage, monitor, and follow-up on client engagement and participation in all treatment plan activities.
- Coordinate discharge planning, and schedule follow-up appointments.
- Provide assistance and help link clients directly to benefits, services, housing, and resources when released.
- Assist with requesting bridge medication requests when clients are released.
- Coordinate with previous and anticipated service providers to ensure jail providers are fully aware of treatment history, and that ongoing providers are aware of inmates’ needs upon release.
- Provide advocacy and outreach; help them to advocate for themselves.
- Educate clients regarding their behavioral health diagnoses and treatment.
- Teach coping, anger management, barrier elimination, and problem solving skills.
- Maintain client charting electronically in accordance with Medicaid, ODMHAS, CARF and professional standards.
- Comply with all agency policies and procedures.
- Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
- May be required to perform telehealth services as determined by Signature Health.
- Adherence and completion of compliance training provided by Signature Health.
- Responsible for and completes all productivity/documentation requirements.
- Participate in all assigned staff meetings and staff development programs.
- Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
- Other duties as assigned.
KNOWLEDGE & EXPERIENCE:
- High School Diploma or equivalent required.
- Valid unencumbered Ohio driver’s license and proof of driver’s insurance required.
- Bachelor’s Degree in related social services or criminal justice field preferred.
- 3+ years of previous case management experience providing service to criminal justice populations or relevant experience preferred.
- CDCA preferred.
- Strong computer skills with Microsoft, Excel, etc.
- Knowledge of the community, community organizations, and community resources.
- Knowledge of ASAM levels of care, ability to assist client in obtaining residential, detox, and supported housing services in the community.
WORKING CONDITIONS :
- Work is normally performed in a typical interior/office/clinical work environment.
- While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
- Possible exposure to blood borne pathogens while performing job duties.
- Requires periods of sitting, standing, telephone, and computer work.
- Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
- Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
- Possible exposure to blood borne pathogens while performing job duties.
- Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
