Summary The Housing Navigator provides case management, outreach, and assessments for individuals and households seeking housing support. This role focuses on connecting clients with resources and services related to housing and other determinants of health.
Job Duties:
- Build and maintain relationships with community partners, service providers, and internal teams.
- Conduct intake and assessments for new referrals.
- Meet with residents to identify challenges and make referrals to programs addressing unhoused needs.
- Document all activities and outcomes electronically in the required systems.
- Ensure compliance with housing navigation initiatives and organizational policies.
- Manage case records and collaborate with external partners
- Support residents during relocation efforts, acting as an advocate and liaison.
- Attend regular meetings with internal and external stakeholders.
- Assist clients with housing challenges such as rent payment, eviction prevention, and documentation recovery.
- Add clients to waiting lists for housing programs
- Maintain up-to-date housing lists and stay informed about city, county, state, and federal funding programs.
- Performs other job-related duties as assigned.
Education, Knowledge, and Skills:
As a Housing Navigator, you must have a high school diploma or equivalent although a bachelor's degree is preferred. Along with at least 2 years of case management experience is required:
- Understanding of social services programs and educational methods.
- Ability to work independently, manage multiple projects, and communicate effectively.
- Strong problem-solving skills and ability to adapt to change.
- Proficient in computer applications and budget management.
- Bachelor's degree in social work or a related field is preferred.
- 2 years of experience in social services.
- Valid Louisiana Driver's License required.
- Ability to sit, stand, and walk for extended periods.
- Capability to lift up to 50 pounds occasionally.
- Perform administrative tasks and occasional travel as needed.
- Must be able to respond within 72 hours to designated locations when requested.
