Job Description:
- Ensure that each client is receiving appropriate and effective case management services
- Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.
- Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
- Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.
- Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients' current needs.
- Provide crisis intervention to clients and their families as needed.
- Maintain ongoing awareness of clients' medical conditions so that services are appropriate to changing needs.
- Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
- Ensure that clients pay the balance of their rent, telephone and utility bills.
- Provide counseling and orientation regarding tenant's rights and responsibilities to maintain and retain housing.
- Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary.
- In collaboration with vocational counselor and entitlement specialist provide advocacy & referrals to employment programs for training and job placement in the private sector.
- Assist clients in locating alternative housing options, when a client's health so requires.
- Provide and coordinate all supportive services as needed.
- Collaborate with all providers within and outside of the organization involved with the client/family. Coordinate and collaborate with HASA personnel as required.
- Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living
- Maintain and complete required documentation including all contacts made with the client and any contact on their behalf
- Participate in weekly supervision
- Submit a weekly schedule of field visits
- Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors
- Bachelor's Degree in Social Work or Public Administration preferred
- Or Associates Degree with 4+ years of related experience
- Or High School Degree with 6+ years of Case Management experience
- Must have 1+ year Case Management Scatter Site experience
- Bilingual Spanish a must
- Individual/Family and Groul counseling experience
- Specifically with HIV+/Aids affected populations and/or substance abuse individuals
