The Case Manager - IOP SUD provides case management services for the Intensive Outpatient Program.

Duties & Responsibilities
- The Case Manager - IOP SUD obtains initial insurance authorizations.
- Obtains extensions for insurance authorizations.
- Check daily attendance and maintain admission log and distribute weekly census report.
- Facilitate groups.
- Document all collateral contacts and services in EHR.
- Meet with clients to develop and review treatment plans.
- The Case Manager - IOP SUD links clients to appropriate services, such as primary care, transportation, and psychiatric.
- When discharge is planned, arrange for aftercare appointments.
- Phone or letter outreach for clients who drop out.
- Participate in team meetings and program supervision.
- The Case Manager - IOP SUD markets the program.
- Participate in ongoing development of curriculum for IOP.
- Provide one-on-one counseling
- Complete discharge summaries/Disenrollment MIS (EIM/ESM).
- The Case Manager - IOP SUD answers phone inquiries about the program from prospective client and referral sources.
- Adheres to the work schedule. Obtains supervisor approval in advance of schedule conflicts as well as late arrivals or early departures.
- Records all hours worked by the last day of the pay period.

Required Experience:
Education & Experience
- BA preferred in Social Work/ related field (or see certifications).
- 2-4 years’ experience
- RSN, CADC, or LADCII required.
- Knowledge of SUD and Mental Health Counseling
- Prior knowledge of health insurance authorizations
