The Case Manager - IOP SUD provides case management services for the Intensive Outpatient Program.

Duties & Responsibilities

• The Case Manager - IOP SUD obtains initial insurance authorizations.
• Obtains extensions for insurance authorizations.
• Check daily attendance and maintain admission log and distribute weekly census report.
• Facilitate groups.
• Document all collateral contacts and services in EHR.
• Meet with clients to develop and review treatment plans.
• The Case Manager - IOP SUD links clients to appropriate services, such as primary care, transportation, and psychiatric.
• When discharge is planned, arrange for aftercare appointments.
• Phone or letter outreach for clients who drop out.
• Participate in team meetings and program supervision.
• The Case Manager - IOP SUD markets the program.
• Participate in ongoing development of curriculum for IOP.
• Provide one-on-one counseling
• Complete discharge summaries/Disenrollment MIS (EIM/ESM).
• The Case Manager - IOP SUD answers phone inquiries about the program from prospective client and referral sources.
• Adheres to the work schedule. Obtains supervisor approval in advance of schedule conflicts as well as late arrivals or early departures.
• Records all hours worked by the last day of the pay period.
• All other duties as requested.

Education & Experience

• BA preferred in Social Work/ related field (or see certifications).
• 2-4 years’ experience
• RSN, CADC, or LADCII required.
• Knowledge of SUD and Mental Health Counseling
• Prior knowledge of health insurance authorizations
