Job Title: Case Manager - Life is Precious™

Responsibilities:

• Provide support and guidance to Latina teens and their families.
• Develop and implement individualized case management plans.
• Facilitate workshops and group activities focused on mental health and wellness.
• Collaborate with community resources and organizations.
• Monitor and evaluate program effectiveness and participant progress.

Qualifications:

• Bachelor's degree in Social Work, Psychology, or related field.
• Experience working with youth and families, preferably in a non-clinical setting.
• Strong communication and interpersonal skills.
• Fluency in Spanish is preferred.
