Essential duties include:

• Complete documentation accurately and in a timely manner
• Conduct patient assessments and ongoing care of IRF/SNF patients
• Communicate with patients, families and co-workers
• Manage casework needs patients and family members
• Participate in discharge planning and in-service programs related to the needs of the patient
• Great time management skills

Qualifications:

• Minimum of 1-2 years of experience as an Licensed Practical Nurse
• Current Louisiana Licensed Practical Nurse LPN license
• Previous Inpatient Rehabilitation or Skilled Nursing Facility experience preferred
• Good oral and written communication skills
