As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.
Requirements:
- Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, Human Services Counseling and 1 year of substance abuse experience providing direct care or 1 year of clinical experience working with individuals with co-occurring diagnoses of SUD and mental health (OR)
- Registered Nurse with 1 year of direct experience providing services to individuals with a diagnosis of substance abuse or 1 year of experience working with individuals with co-occurring diagnoses of SUD and mental health (OR)
- CSAC or CSAC-Assistant
- Valid/Current Driver's License
- Ability to travel up to 25% of the time
- Ability to work overtime as needed
Responsibilities:
- Providing case management and care coordination services for clients/patients enrolled in treatment. Responsible for coordinating continuum of care activities for assigned clients/patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.
- Conducting individual and family educational sessions and engaging in various duties related to coordinating treatment and discharge activities, as needed.
- Coordinating, in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu.
- Adheres to organizational policies, procedures, performance improvement initiatives and maintains organizational and industry policies regarding confidentiality.
- Performing other duties as assigned.
