As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Clinical Director and Executive Director to establish and maintain a therapeutic environment and routine for our residents.
Requirements:
- Bachelor’s degree preferred in the Human Service field from an accredited college or university.
- Previous experience in case management and/or utilization review. (preferred)
Responsibilities:
- Assess new referrals for appropriate level of care referral.
- Determines need for therapeutic medical, psycho-social, and psychiatric evaluations.
- Coordinates services with counselors and medical case managers.
- Monitors cases by verifying patient attendance, observing and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises, and providing widespread support.
- Assist with discharge plans and aftercare referrals.

Experience
Preferred- · Previous experience in case management and/or utilization review preferred.
Education
Required- Associates or better in Human Services
- Bachelors or better in Human Services or related field
