At our 40-bed hospital, we provide specialized inpatient and outpatient rehabilitative services to our patients, serving Colorado, Wyoming, and Western Nebraska.
We are seeking a Case Manager to join our team of passionate patient caregivers!

Required Skills:
- Current clinical licensure strongly preferred.
- One year recent relevant experience required. Acute care setting preferred.

- Knowledge of community resources to meet post-discharge clinical and social needs.
- Knowledge of clinical operations and procedures.
- Ability to establish and maintain pro-active relationships with internal interdisciplinary team members, local payer, physicians, community service organizations and health care facilities.
- Ability to maintain quality, safety, and/or infection control standards.
- Demonstrates general computer skills including: data entry, word processing, email, and record management.
- Effective organizational and time management skills.
- Effective written and verbal communication skills.
- Ability to maintain proper levels of confidentiality.
- Ability to work closely and professionally with others.
Responsibilities:
Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital’s mission and “Guiding Principles” into daily practice.
Posted Total Compensation :The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. It’s not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $38.00/hr.Minimum Salary:USD $38.00/Hr.Maximum Salary:USD $38.00/Hr.
