Job purpose
Provide case management, psychoeducation, and clinical documentation under the supervision of the Residential Supervisor.
Duties and responsibilities
- Aid patients in obtaining necessary services, such as housing assistance, food, clothing, medical services, educational services, vocational services, etc.
- Develop and implement individualized treatment plans, and other clinical documentation.
- Build and maintain positive relationships with referral agencies.
- Able to maintain a minimum of 50% productivity requirements.
- Facilitate psychoeducation groups using provided material.
Qualifications
Certified by the Ohio Chemical Dependency Professionals Board. Previous experience is not required but prefer one-year paid experience in case management. Basic computer and Microsoft office skills are required.

Working conditions
Reports to the Residential Supervisor. Supervision occurs through regular individual supervisory sessions, participation in group supervision meetings, daily census report, and review of compliance with clinical performance standards and measures through chart audits.
Work Schedules and job functions are subject to change relative to the needs of the organization.
