Position Summary:
Under the direction of the Rapid Rehousing Case Manager Supervisor, the Rapid Rehousing Case Manager is responsible for providing home-based case management services for families who have recently experienced homelessness. The primary goal is to assist families in addressing the barriers to maintaining permanent housing using a strengths-based, Housing First approach. Functions performed include housing location, family goal planning, connection to community resources, home visits, rental payment monitoring, landlord engagement, and problem solving with other case managers. Work is accomplished by advising and motivating program participants to accomplish goals in their Family Service Plan.
Highlighted Responsibilities and Duties
- Serves as the case manager and service coordinator for assigned participants.
- Facilitates and ensures connection to needed Community Services and work in collaboration with Community Agencies to ensure effective communication and participant engagement.
- Connects assigned participants to and ensure their engagement in needed supportive services that will address barriers and challenges they face.
- Track participant's engagement/ participation in supportive services, workforce development, employment training
- Completes all required case notes, housing stabilization plans, documents, files, and assigned reports related to participants/case activity.
- Develops and maintains individual participant's records/files that comply with all federal laws as well as DHS/FSA program standards.
- Complete comprehensive monthly budgets with participants
- Input participant data and program activities into designated software database as assigned.
- Complete documentations for participants Quarterly Recertification
- Prepare and issue all Notices to participants i.e. Exit, Termination, Extension Notice
- Enters participant housing information into HMIS within 72 hours of lease –up.
Qualifications
- Bachelor’s degree in social work, psychology, sociology, counseling, or related service/science or health care related disciplines
- Minimum of one year of case management experience
- Knowledge of community resources
- Excellent communication skills
- Ability to maintain confidentiality or records and information
- Ability to adapt to changing needs of clients
- Ability to work with diverse personalities, persons of all ages, ethnicity, religion and sexual orientation
- Proficiency in MS Office and excellent administrative skills
- Strong interpersonal and organization skills required;
- Strong communication skills including verbal and writing;
- Proven ability to submit timely, detailed and accurate case notes.
