Our client is currently seeking a Case Manager RN to work on a transition of care team
The location of work will be both remote and on the site days will be at the Soper Hill clinic (Marysville/Lake Stevens)
This job will have the following responsibilities:
- Hospital Transition calls for Medicare Advantage patients
- Care Coordination for complex, high-risk patients
- Motivational interviewing with patients and families
- Palliative Care conversations when appropriate
- Participation in Interdisciplinary Team meetings
Qualifications & Requirements:
- Ambulatory Care
- Outpatient Care Coordination
- Difficult conversations
- Care Management
- Hospital Transition work
- WA State RN license
Start & end times of shift: 7:30-4 pm
Days of the week they'll work: M-F
