Job Details
We are looking for a proactive and skilled Case Manager to coordinate resources and services, ensuring optimal patient outcomes while managing fiscal realities.
Key Responsibilities:
Coordinate patient care from admission to discharge, ensuring timely and appropriate clinical interventions.
Track and analyze clinical service utilization for specific patients to ensure proper timing and effectiveness of care.
Provide analysis and interpretation of findings to nurses, multi-disciplinary teams, medical staff, and managers.
Support decision-making regarding health plan coverage, patient outcomes, and overall hospital costs.
Ensure services are delivered in a timely manner, impacting the length of stay and appropriateness of treatment.
Advocate for the triple aim: improving the patient experience, enhancing patient outcomes, and managing the cost of care.
Collaborate with healthcare teams to monitor and improve the quality of care provided to patients.
Qualifications:
Associate Degree in Nursing, required; Bachelor of Science Degree in Nursing, preferred
Current and in good standing SC RN license with SCLLR.
Minimum two years RN Case management experience in health care delivery acute care setting preferred.
Knowledge in acute care level psychosocial intervention and practices required.
