Job Summary:
The RN Care Manager is responsible for performing comprehensive assessments on patient admissions and facilitating the care coordination process. The role focuses on high-risk patient populations to ensure appropriate discharge planning and prevent readmission. The RN Care Manager collaborates with healthcare professionals and communicates vital patient information to ensure optimal care outcomes.
Essential Functions and Responsibilities:- Commercial payer reviews
- Disposition determination
- Retro/self-audit
- MC variance
- Admission clinical review
- Registered Nurse (RN) NYS license required.
- BLS certification required.
- COVID-19 vaccination required.
- PRI certification to be obtained within three months of initial employment.
- Screen certification strongly preferred.
- Minimum of three years of clinical experience in Emergency Department, critical care, or medical/surgical settings preferred.
- Previous case management experience preferred.
- Knowledge of healthcare financial, regulatory, and payer issues preferred.
- Understanding of state, local, and federal programs preferred.
- Experience using Milliman/InterQual criteria preferred.
- Strong computer skills are essential.
