About The Position
**Job Title:** Registered Nurse **Job Specialty:** Case Manager **Job Duration:** 14 weeks **Shift:** Day 5x8-Hour (08:30 - 17:00) **Guaranteed Hours:** Not specified **Experience:** Minimum 1 year **License:** GA or Compact license **Certifications:** BLS **Must-Have:** Ability to assess, evaluate, and drive discharge plan **Job Description:** - Coordinate services to the patient's appropriate level of care. - Ensure that treatment plans are initiated, evaluated, and re-evaluated daily. - Follow daily progress and address any barriers to discharge. - Monitor the length of stay and identify potential barriers to discharge. - Adhere to Medicare regulatory guidelines regarding patient choices, IMM notices, and MOONS. - Participate in a pre-screening unit; candidates will be reviewed to determine clinical fit. - Work requirement on holidays: - If one major holiday falls within the assignment period, the traveler is expected to work it. - If two major holidays fall, the traveler is expected to work both. - If three major holidays fall, the traveler is expected to work two of them. - Major holidays include Christmas Eve and Day, New Year's Eve and Day, Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday. - Compliance requirement: Full compliance required to start on time. Please ensure the completion and scheduling of necessary compliance items within 48 hours of signing the contract to ensure an on-time start.
