About The Position
**Job Title:** Registered Nurse **Job Specialty:** Case Manager **Job Duration:** 13 weeks **Shift:** Day 2x8-Hour (08:00 - 16:30) **Guaranteed Hours:** Not specified **Experience:** Minimum of 2 years in care coordination and utilization management required **License:** Valid KY or compact license required **Certifications:** Must have EPIC experience; Copy of certifications required or must be willing to obtain before starting **Must-Have:** Candidates need both Care Coordination and Utilization Management experience **Job Description:** - Identify, initiate, and manage optimal patient flow/throughput to enhance continuity of care. - Plan and facilitate coordinated, safe transitions to the next level of care required. - Promote patient satisfaction and quality outcomes. - Target an optimal length of stay based on the patient's individual response to treatment, procedures, and interventions. - Maximize contracted benefits and available services in care management planning. - Provide comprehensive care management planning. - Work schedule includes weekends, specifically Saturday and Sunday, two days per week until the end of 2024. - Scrubs/Apparel should not have any agency or brand logos on them. This policy is strictly enforced. - Candidates are expected to be able to float within their scope as per job description. **Additional Information:** - COVID-19 Vaccine is required with medical/religious exemptions only. - Flu Vaccine requirement is currently unknown. - Background policy allows the facility to decide whether to move forward with background and license evaluations. - Return staff must be away for a year from full-time, part-time, or per diem roles to be considered for traveling positions. - Candidates with ongoing or recent (last six months) applications for permanent roles within the healthcare facility will not be considered. **Orientation:** 16 hours (Non-Billable)
