**Job Title:** Registered Nurse (RN) - Case Manager **Job Specialty:** Case Management **Job Duration:** 13 weeks **Shift:** Monday to Friday, 08:00-16:30 **Guaranteed Hours:** Full-time, 40 hours per week **Experience:** 2-3 years of case management experience preferred **License:** Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) **Certifications:** Relevant nursing certifications as required **Must-Have:** Licensed in the respective state; proficiency in using EPIC Electronic Medical Records (EMR) **Job Description:** - Provide specialized case management services in an acute rehabilitation setting - Collaborate with a multidisciplinary team to develop and implement patient care plans - Manage patient caseloads effectively to ensure quality of care and compliance with facility standards - Facilitate communication between patients, families, and healthcare providers - Coordinate discharge planning and post-discharge care to ensure continuity of patient care - Maintain meticulous documentation and records in the EMR system - Adhere to business casual dress code policies - Demonstrate exceptional problem-solving and critical thinking skills This position offers an immediate start opportunity and requires flexibility to support the expansion of case management services in the rehabilitation unit.
