Overview
We are seeking a dedicated Case Management Manager to join our team. The ideal candidate will have a strong background in case management and program management.
Responsibilities
Lead and oversee the Extended Case Management (IAR, ICM, ECM and others) programs, including day-to-day operations, staffing, and case management activities.
Supervise program staff, including onboarding, providing regular supervision, coaching, and professional development.
Maintain up-to-date knowledge of ORR Preferred Community guidelines, CalAIM Enhanced Care Management, and Community Supports regulations, and others to ensure program compliance.
Ensure programs and staff strictly adhere to HIPPA, PHI and PII requirements.
Support the development and execution of short/ long-term program strategies.
Requirements
MUST have a Masters in Social Work (MSW)
Associate Clinical Social Worker (ASW) working toward licensure -OR- 2 years active unrestricted license in California as a Licensed Clinical Social Worker (LCSW) required with experience providing clinical supervision
2+ years program administration experience
Minimum of 4 years in Case Management, health education, or related fields
