Job Description and Responsibilities
The Case Manager assists in the effective case management of the census of the clinic they operate at. This includes overseeing day-to-day clinical case management for client-driven beneficial outcomes.
The role consists of:
Assessment: Conduct initial assessments to determine the client's mental health or substance abuse needs, as well as their strengths and limitations.
Treatment Planning: Collaborate with clients and other healthcare providers to develop personalized treatment plans, outlining goals and interventions.
Coordination of Care: Coordinate and facilitate communication between the client, healthcare providers, social services, and support networks to ensure comprehensive care.
Referrals: Make referrals to mental health professionals, substance abuse counselors, support groups, and other relevant services based on the client's needs.
Advocacy: Advocate for clients' needs within the healthcare system, ensuring they receive appropriate services and support.
Crisis Intervention: Provide immediate support and intervention during mental health or substance abuse crises, including coordinating emergency services when necessary.
Education: Educate clients and their families about mental health and substance abuse disorders, treatment options, and available resources.
Monitoring Progress: Regularly assess clients' progress toward their treatment goals and modify the treatment plan as needed.
The Case Manager is additionally responsible for the following:
- Complete initial case management assessments
- Case management intake process
- Develop appropriate individual case management plans with clients
- Provide ongoing case management functions in accordance with the needs and goals identified
- Maintain communication and collaboration with client’s support system
- Provide ongoing face-to-face reviews with clients to assess the progress made in reaching goals so that the treatment plan can be modified as necessary to ensure that the goals and objectives are being achieved
- Document all interactions, including face-to-face, family communication, and probation updates.
- Demonstrate appropriate & complete documentation of treatment-related information, including case management notes, progress notes, correspondence, releases of information, etc.
- Facilitate case management groups
- Participate in regular interdisciplinary staff meetings
- Maintain collaborative relationships and communication with community referring agencies and community partnerships
- Implementation and education of life skills with clients individually - life skills criteria include money management, time management, medication management, goal setting and planning, communication, and interpersonal skills, and decision-making and problem-solving
- Perform other duties as assigned by the supervisor, which are consistent with the position and in compliance with policies and procedures
- Meetings and continuing education are included as a requirement of this role.
POSITION QUALIFICATIONS
- High School Diploma or GED
- Minimum of 12 months experience in working in the field of substance abuse treatment or behavioral health-related field; case management experience preferred
- Must be at least 21 years of age
- Active, valid driver’s license
- CPR certification
- Complete all required documentation in accordance with company policies and state criteria
