We are looking for individuals who have a passion for helping others to work with clients in the Lewiston, Maine area. You MUST have a MHRT-C certification for this position.

Job Overview:
As a Case Manager, you will assist clients in finding services, programs, and resources to help them gain independence and work towards their established goals; while also providing support, advocacy, referral, and assessment services.
- Understand each client's diagnosis and recommend services based on individual assessment.
- Serve as a liaison between clients and all agencies providing services, programs, and community resources. Obtain all necessary releases to provide or receive information as needed.
- Meet with other service providers to plan and coordinate treatment to ensure the integration of the treatment across the clients home, school, and community.
- MUST have an MHRT-C certification and bachelor's degree
- At least 1 year of experience providing social services
- Clean background check
- Must be organized, detail oriented, and able to work independently in the field
