Dwyer Workforce Development's (DWD) mission is to provide comprehensive support to individuals who lack opportunity and aspire to pursue a career in the healthcare industry, alleviate a severe healthcare workforce shortage, and improve the lives of seniors. We are a Baltimore, MD based NFP hiring in the Rio Grande Valley, TX area.
Position Summary: Provide case management duties and coordination of comprehensive support and services to Dwyer Scholars.
Position Responsibilities:
• Develop a good working knowledge of agency mission, vision, and philosophy.
• Engage with applicants and collect data to determine program eligibility based on DWD policies, application information provided and required documentation.
• Maintain follow-up contact as required, with participants through telephone, mail, e-mail, in-person visits, and text messages.
• Responsible for thoroughly and accurately documenting service encounters with Dwyer Scholars, Trainers and Provider/Employer Partners, activities, and interventions as well as demographic information for improvement and evaluation.
• Maintaining current and confidential record keeping systems including updating contacts notes and tracking the progress of Dwyer Scholars in meeting training and employment goals.
• Work with Dwyer Scholars to identify barriers to success; use self-sufficiency development tools and resource solutions and action plans to eliminate barriers.
• Provide direct services to Dwyer Scholars in crisis situations – both on-site and off-site – including supportive listening, community and resource referrals, goal-setting and appropriate follow-up.
• Secure employment opportunities for Dwyer Scholars through job development activities and effective communication with Provider/Employer Partners.
• Assist Dwyer Scholars in developing and honing job-seeking and workplace readiness skills in preparation for successful employment.
• Provide follow-up services to support job retention, as well as ongoing engagement
opportunities and support to facilitate independence, assisting Dwyer Scholars in maintaining life balance.
• Must maintain client confidentiality and abide by the Data Privacy Act.
• Demonstrate teamwork by cooperating with and assisting co-workers as needed.
• Attend required meetings, training, seminars, and conferences as needed/required to enhance expertise and professionalism in agency activities as scheduled by the Director and/or funding source, outside agencies, etc. to fulfill job/project(s) requirements.
• Must be responsible and able to handle confidential material and information, organize effectively, and work independently.
• Other duties as assigned.
Essential Qualifications:
• Knowledge of local community resources and social services in applicable
geographic area.
• Bachelor of Arts or Science in social work or human services or related
equivalent experience. Previous experience working with human service
agencies/programs preferred.
• Possess highly developed critical thinking skills, strong verbal and written
communication skills as well as leadership and motivational skills.
• Proactive qualities with a willingness to both contribute and learn.
• Strong time-management skills and an ability to organize and coordinate multiple projects.
• Proficiency with office productivity tools and an aptitude for learning new software and systems.
• Effective organizational development and critical thinking skills.
• Knowledge of Salesforce or other CRM.
• Ability to work flexible hours and evenings, when necessary.
• Ability to establish and maintain effective working relations with a diverse population.
• Bilingual preferred.
License/Certification: Driver's license and access to a vehicle required.
