Qualifications
Master's degree in social work, psychology, counseling or a related field and experience working with individuals with severe and persistent mental illness. Able to work one evening night per week (10:00 AM - 6:00 PM). Valid driver’s license in good standing.
Responsibilities
- Provide on-going monitoring of individuals in the AOT program to ensure compliance with individualized court order.
- Conduct intake assessments, develop and oversee treatment plans for individuals involved in AOT program.
- Oversee implementation of program services and procedures.
- Communicate observations and information regarding individuals on your caseload to the collaborative team consisting of AOT Director, AOT Psychiatrist, and treatment team.
- Provide case management to individuals on your caseload including, but not limited to, linkage and service coordination to individuals to appropriate/court order treatment modalities, assist with obtaining public entitlements and benefits such as Social Security, Medicaid, Medicare, NJSNAP, supportive housing, and other government assisted programs.
- Supervise, organize and support recreational and/or socialization activities.
- Provide transportation, support counseling, and crisis intervention services as needed.
- Document observations, information, and services utilizing an Electronic Health Record (EHR).
- Participate in staff meetings, treatment planning, relevant trainings, and agency orientation.
- Establish and maintain an ongoing therapeutic relationship and case load as skill level permits.
- Monitors service delivery to meet program participants' changing needs.
- Performs other duties as assigned by the AOT Director.
